To request a certificate of Baptism, First Communion, Confirmation or Marriage, complete the Sacrament Certificate Request form. Please note the following:
You may only request your own certificate, or for your child under age 18.
All certificates must be picked up at the parish office in person, with valid ID. We cannot mail, email or fax certificates to you personally.
Out-of-town requests will be mailed to your local Catholic parish, where you can pick them up in person with valid ID. Please provide the parish name, address and phone number for delivery.
